Complete Blood Count (CBC) Test
The most commonly ordered blood test there is, and the one that most often points to what to look at next.
A complete blood count measures the cells circulating in your blood: red cells that carry oxygen, white cells that fight infection, and platelets that clot. It is the most frequently ordered blood test in medicine, because it is inexpensive, fast, and abnormal results narrow the field quickly.
A CBC rarely gives a diagnosis by itself. What it does well is tell a doctor where to look. A low haemoglobin points towards anaemia and its causes, a raised white count towards infection, an abnormal platelet count towards bleeding or clotting problems.
At a glance
The main components and what an abnormal value suggests.
| Vaccine or test | Status | Schedule | Detail |
|---|---|---|---|
| Haemoglobin (Hb) | Routine | Part of every CBC | Oxygen-carrying capacity. Low haemoglobin means anaemia, which is very common in India, particularly among women and adolescent girls. The CBC also hints at the cause through red cell size. |
| Red blood cell indices (MCV, MCH) | Routine | Part of every CBC | Red cell size and haemoglobin content. Small pale cells point towards iron deficiency; large cells point towards B12 or folate deficiency. This is what makes a CBC more informative than a haemoglobin reading alone. |
| White blood cell count (WBC) | Routine | Part of every CBC | Raised counts usually indicate infection or inflammation. The differential breakdown separates bacterial patterns from viral ones, and flags less common causes. |
| Platelet count | Routine | Part of every CBC | Clotting cells. A falling platelet count is a recognised feature of dengue, which makes this one of the more closely watched values during monsoon season in India. |
| Haematocrit (PCV) | Routine | Part of every CBC | The proportion of blood volume made up of red cells. Read alongside haemoglobin, and affected by hydration. |
Why anaemia turns up so often

Anaemia is one of the most common findings on a CBC in India, and it is frequently present without obvious symptoms. Tiredness gets attributed to workload, breathlessness on stairs to being unfit, and the underlying deficiency goes unaddressed for years.
The CBC does more than detect it. Red cell indices suggest the type: iron deficiency produces small, pale cells; B12 or folate deficiency produces large ones. That distinction changes what is investigated next and what treatment is appropriate, which is why treating a low haemoglobin with iron tablets without knowing the cause is not always right.
Reading your own report sensibly
Laboratory reports flag anything outside the reference range, which leads a lot of people to worry about values that are barely outside and clinically unimportant. Reference ranges are statistical, not diagnostic: they describe where most healthy people fall, and healthy people sit outside them all the time.
What matters more is the size of the deviation, the pattern across several values, whether it is changing over time, and whether it fits your symptoms. That is a judgement, which is why we would rather go through a report with you than hand it over.
When a CBC is worth doing
It is part of most general health checkups, and specifically worth doing in these situations.
- Persistent tiredness, breathlessness or pallor.
- Fever, especially during dengue season when platelet trends matter.
- Before surgery or any procedure.
- During pregnancy, as part of routine monitoring.
- Heavy menstrual bleeding, a common cause of iron deficiency.
- Monitoring a known condition or a medication that affects blood counts.
- As a baseline in a routine health checkup, so future results have something to compare against.
Frequently asked questions
Do I need to fast for a CBC test?
No. A complete blood count does not require fasting. If it is being done alongside a fasting glucose or lipid profile, follow the fasting requirement for those tests.
What does a CBC test detect?
Anaemia and its likely type, signs of infection or inflammation, and platelet abnormalities affecting clotting. It rarely diagnoses a condition outright, but it narrows down what to investigate next.
What does a low haemoglobin mean?
Anaemia, which has many causes. Red cell indices in the same report suggest which: small pale cells point towards iron deficiency, large cells towards B12 or folate deficiency. The cause determines the treatment, so it is worth establishing rather than assuming.
Why is the platelet count watched during dengue?
Dengue characteristically lowers platelet counts, and the trend over successive days matters more than any single reading. This is why repeat CBCs are often ordered during a dengue illness.
My report has values marked outside the range. Should I be worried?
Not necessarily. Reference ranges describe where most healthy people fall, so healthy people sit slightly outside them routinely. What matters is how far outside, the pattern across values, whether it is changing, and whether it fits your symptoms.
